Greater Roslindale Medical and Dental Center
Greater Roslindale Medical & Dental Center hired ICH to do an assessment of the current state of their complex care management program, using both quantitative and qualitative data. We identified best practice recommendations from other programs for how to identify and engage patients, and provided recommendations for how to use data for ongoing program evaluation. The assessment included a literature review and synthesis of findings related to patient identification and engagement in complex care programs, interviews with GRMDC staff, and an analysis of patient-level data to characterize the population served by the program. We also engaged with the GRMDC staff to develop a logic model and provided recommendations for GRMDC as they build their evaluation infrastructure.
