Complex Care Management Impact Evaluation

Cambridge Health Alliance’s Complex Care Management (CCM) program aims to bend the acute utilization curve of CHA’s highest-cost and most vulnerable patients by providing additional support through care management teams. ICH has worked with CCM for four years, first to support a process evaluation to document how the program works and to gather preliminary data on outcomes. We are currently implementing a rigorous impact evaluation using linked medical records and claims data to create a nuanced understanding of patients’ healthcare utilization and to determine if participation in the CCM program reduces acute utilization and total medical expenditures.
